I'm a Teacher eligible for the incentive. I have questions about how to fill out the W-9 form. Can you help?

The RecognizeB5 Team at VECF will reach out to eligible teachers directly to complete the Opt-In and W9 form necessary for payment processing.  W-9 information will not be shared and all personal information will be kept secure and confidential.

NOTICE: Beginning July 1, 2024, our secured online system for accepting W9 Forms

for RecognizeB5 WILL BE CLOSED.

Teachers needing to submit and/or update a W9 Form after June 30th will need to MAIL completed forms to the VECF Office. A blank copy of the W9 form and mailing instructions can be found here. Teachers needing assistance should contact RecognizeB5 for support.

Most EDUCATORS will complete this form as an INDIVIDUAL and not as a business. As an individual, you will complete boxes 1, 3a, 5, and 6. Boxes 2 and 7 are not necessary. Box 2 (Business Name) should only be completed if you are a business owner and the payment should be made to your Business.  It is NOT intended for the name of the Childcare Program where you are employed.

The IRS also has a Spanish version of the full IRS W9 with specific line-by-line instructions.

 

Some suggestions for accessing and completing the W9 form online:

  • If you are using a mobile device, tap directly to the top right of the phrase "Please enter your individual access code" to display the access code box. If you are using a computer or tablet the code box will appear in the center illuminated in teal.  
  • Codes are case sensitive and must be typed as shown or Copy the original code, click into the platform through the link provided and paste it into the code box. You will not use any spaces or punctuation. 
  • Browsers suggested for accessing the platform are Chrome and Internet Explorer.  The platform can be accessed from a mobile device, computer or tablet.
